The primary objectives of the Primary Impact Initiatives project are:

1. Improve Access to Education — Enhance educational opportunities for disadvantaged children, promoting literacy, numeracy, and life skills.

2. Promote Health and Wellbeing — Provide access to healthcare services, promote healthy lifestyles, and support mental health initiatives.

3. Foster Economic Empowerment — Support entrepreneurship, job creation, and financial inclusion initiatives to reduce poverty and inequality.

State Cold Chain Officer (CCO)

Disease Control and Immunization

Ensures effective vaccines and logistics management across all immunization facilities in Adamawa State, maintaining vaccine cold chain integrity from state level to last-mile delivery points.

The empowerment programme supports Community Health Influencers, Promoters and Services (CHIPS) agents to be self-sufficient — enabling them to engage in community contributions and sustain income-generating businesses alongside their health promotion duties.

Community Health Services

The MNCH programme drives three core outcomes for communities across Adamawa State:

  • Encourage health facility deliveries to increase skilled birth attendance
  • Encourage pregnant women to regularly visit Antenatal Care (ANC) clinics
  • Encourage Postnatal Care (PNC) follow-up at Day 1, Day 3, and Week 6 after delivery

Adolescent Youth Sexual Reproductive Health (ASRH)

Community Health Services

Adolescent Sexual and Reproductive Health (ASRH) is the ability of adolescents to make informed decisions about their health and bodies, and to access the services and education they need to do so. It is critical because it helps improve overall wellbeing and protects adolescents from serious challenges like sexual violence, intimate partner violence, and child marriage.

The programme addresses key challenges including: lack of education and information, limited access to health services (especially contraception), gender inequalities and harmful traditional practices, risk of STIs including HIV, and social stigma.

Programme Objectives: Educating young people on reproductive health; preventing STIs and HIV; empowering young people to know and exercise their rights; reducing negative outcomes such as early pregnancy; improving overall wellbeing; and protecting young people from harm.

The Health Education programme pursues two core objectives:

Objective 1 — Improve the health of individuals and communities by promoting healthy practices, knowledge, and behaviours.

Objective 2 — Inform the general public about physical and mental hygiene and methods of preventing avoidable diseases through targeted education campaigns and community outreach.

Planning Research and Statistics

DHIS2 (District Health Information System 2) is a free, open-source platform for collecting, analyzing and sharing health data. It is the world's largest health management information system (HMIS) platform and is used to collect, analyze and visualize health data at facility, LGA, state, and national levels.

ADSPHCDA uses DHIS2 as its central data platform for tracking programme indicators, facility-level reporting, disease surveillance, and health outcome monitoring across all 21 Local Government Areas of Adamawa State.

ALMANACH

Planning Research and Statistics

ALMANACH is a digital clinical decision support system based on the Integrated Management of Childhood Illness (IMCI) guidelines. It serves as an electronic tool for health care workers to improve the quality of under-5 consultations, especially in underserved communities.

The project has been implemented in Adamawa State since 2016 through a collaborative effort between ADSPHCDA, ICRC, and the Swiss Tropical and Public Health Institute (SwissTPH), with the aim of promoting adherence to standardized clinical guidelines and evidence-based practice.

ALMANACH is currently active in 413 health facilities in Adamawa State and is being piloted in six additional states: Kano, Kaduna, Borno, Yobe, Taraba, and Gombe.

Published outcomes include: 80% reduction in antibiotic prescriptions, substantially improved diagnostic accuracy, improved caregiver-reported recovery rates, and increased adherence to clinical protocols. The project received the National Council on Health's approval at its 63rd meeting in 2022.

Essential Medicines Supply Chain

Essential Drugs and Logistics

Responsible for the procurement, storage, and distribution of essential medicines and medical supplies to all Primary Health Care facilities across the 21 LGAs of Adamawa State. The programme ensures that every health facility maintains adequate stock levels of WHO and NPHCDA essential medicines at all times.

Key Functions: Coordinating procurement through the Adamawa State Drug Revolving Fund; managing state medical stores; ensuring last-mile delivery to health facilities; monitoring stock levels and expiry dates; and supporting LGA pharmacists and store keepers.

Cold Chain and Vaccine Logistics

Essential Drugs and Logistics

Manages the vaccine cold chain system from state level down to last-mile health facilities, ensuring vaccines remain potent and safe at all points of the supply chain. The programme coordinates maintenance of refrigerators, cold boxes, vaccine carriers, and ice packs across all facilities.

Scope: State Vaccine Store management; LGA cold chain equipment maintenance schedules; temperature monitoring and AEFI reporting; solar refrigerator programme for off-grid facilities; and capacity building for cold chain handlers at all levels.

Medical Supplies and Equipment

Essential Drugs and Logistics

Coordinates the procurement and distribution of medical equipment, consumables, and supplies including rapid diagnostic test kits (RDTs), Long-Lasting Insecticidal Nets (LLINs), ORS sachets, zinc tablets, iron/folic acid supplements, and other programme-specific commodities.

Partnership: Works in close collaboration with UNICEF, WHO, Global Fund, and the Adamawa State Ministry of Health Supply Chain Unit to ensure uninterrupted commodity availability at all facilities.

Drug Revolving Fund (DRF)

Essential Drugs and Logistics

The Adamawa State Drug Revolving Fund is a community-based financing mechanism that ensures the sustainable supply of essential medicines at the PHC level. Health facilities collect modest user fees for medicines and revolve the proceeds to maintain stock levels.

Objective: To maintain at least 90% availability of tracer essential medicines at all functional health facilities; reduce out-of-pocket expenditure for patients; and eliminate stock-outs through predictive quantification and timely resupply cycles.
